NEW/LOST PASSPORT
NEW US PASSPORT REQUIREMENTS EFFECTIVE MARCH 8, 2005
PROCEDURE FOR THE FIRST TIME PASSPORT APPLICATION
(for individuals who have never had a passport, lost a valid passport, or whose passport was issued more than 15 years ago
or prior to their 16th birthday, or whose most recent passport is damaged or mutilated)
You will need to submit the following items:
• Certified birth certificate, which was issued by either state or country, along with its raised seal, or naturalization
certificate, or passport issue over 15 years ago.
• Two recent professional passport photos (taken no longer than 3 months).
• Completed brown passport application form DS-11;Please make sure that this form was filled out completely but not
signed. The postmaster or clerk court house must witness your signature. Mailing address must be left blank on
passport forms.
• Proof of departure – computer generated flight itinerary from the airline, copy of airline ticket, or a corporate letter
stating US departure by private aircraft (require for same/next day processing).
• Letter from United Travel Center to the Postmaster/ Clerk Courthouse for the hand carry service.
• Bring your check book or you can buy money order at the post office.
• You must present proof of identity such as a driver’s license (which has been issued for at least 6 months). All
applicants 1 year or older must appear in person at the post office or the courthouse. Minors must be accompanied by
BOTH PARENTS or legal guardian with acceptable proof of ID.
• A check made payable to the US Dept. of State in the amount of
$135.00 for adult (Application $75 + Expedited $60) and
$120.00 for children under 16 (Application $60+Expedited $60). DS-3053
• Another $25.00 execution fee will be paid at the post office or clerk courthouse.
Present all the above mentioned items to the clerk at the authorized post office or courthouse where your signature will be
witnessed. Ask the clerk for "Hand Carry" Service. The clerk will seal the above materials in an official envelope marked
TO BE OPENED BY PASSPORT OFFICE PERSONNEL ONLY. Do not open this sealed envelope or the application will be
invalid. Important: Make sure the agent returns the sealed envelope to you. If the agent does not feel at ease about
releasing the envelope, suggest that he/she refer to page 31 or 44 of their Passport Agent's Manual, "Hand-carrying of
Executed Applications". The sealed envelope is considered "expired" by the Passport Agency after 5 business days. Please
bring the sealed envelope back to UNITED TRAVEL CENTER for processing the same day/ next day it is signed.
For those who lost a valid passport, must complete lost passport form DS-64
In a separate envelope, please enclosed:
• A signed authorization letter.
• A check payable to United Travel Center $160.00 for up to 7-10 days process

Passport is renewable if:
You can submit your most recent US passport.

You were issued your most recent US passport less than 15 years ago.
You use the same name as on your most recent US passport, OR, you have had you name changed by marriage or court
order and can submit proper documentation to reflect your name change.
If you answer yes to any of the above statements, then you will need to submit the following items:
A check made payable to United Travel Center $160.00 for up to 7-10 business days process.
